Cracked Tooth Syndrome: Why Does My Tooth Hurt When I Bite?

Have you ever bitten down on a piece of granola or a simple almond and felt a sudden, sharp zap of pain shoot through your tooth? It catches you off guard. You stop chewing, wait a second, and bite again—but the pain is gone. You might ignore it for a few days, but then it happens again. It isn’t a constant toothache, but it is unpredictable enough to make you nervous about eating your favorite foods.

If this sounds familiar, you aren’t imagining things, and you certainly aren’t alone. As a dentist, I see this scenario in my practice quite often. You are likely dealing with something we call Cracked Tooth Syndrome (CTS).

It can be a frustrating condition because the crack is often too small to see in the mirror, and sometimes it doesn’t even show up on a standard X-ray. However, the discomfort is very real. I want to walk you through exactly what is happening, why it hurts, and most importantly, how we can fix it to get you smiling and chewing comfortably again.

What Exactly is Cracked Tooth Syndrome?

To understand CTS, we have to look at the anatomy of your tooth. Your teeth are incredibly strong, but they are not indestructible. A tooth is made up of layers. The outer layer is the enamel, which is the hardest substance in the human body. Under that is the dentin, a softer layer that has tiny microscopic tubes leading to the center of the tooth. The center is the pulp, where the nerves and blood vessels live.

Cracked Tooth Syndrome happens when a tooth has a tiny crack that usually extends from the chewing surface down toward the root. Unlike a broken bone that might heal on its own, a cracked tooth does not heal. In fact, without treatment, the crack will usually slowly spread.

Think of it like a crack in a car windshield. It might start as a tiny chip, but as the car vibrates and temperatures change, that crack starts to run across the glass. Your tooth is under similar pressure every single day.

Why Does It Hurt Only When I Bite?

This is the most common question I get asked. Patients often tell me, “Dr. Vafa, my tooth feels fine when I’m not eating. Why does it only hurt when I chew?”

The answer lies in physics. When you bite down, the pressure causes the segments of the tooth to flex slightly apart. When you release the bite, those segments snap back together. This movement does two things:

  1. It irritates the nerve inside the pulp.
  2. It causes fluid inside the microscopic tubes of the dentin to move, which triggers a sharp pain sensation.

It is distinct from a cavity, which usually causes a dull, throbbing ache. CTS pain is typically sharp, erratic, and happens quickly upon biting or releasing bite pressure.

Common Symptoms to Watch For

Because the crack can be invisible to the naked eye, you need to pay attention to what your mouth is telling you. Here are the classic signs I look for:

  • Erratic Pain: Pain when chewing, especially when releasing bite pressure.
  • Temperature Sensitivity: Sudden pain when eating or drinking something hot or cold.
  • Sweet Sensitivity: Sometimes sugary foods can trigger a reaction in the spot.
  • Difficulty Pinpointing the Tooth: You might know the pain is on the “bottom left,” but you can’t figure out exactly which tooth is the culprit.

The Different Types of Tooth Cracks

Not all cracks are created equal. In dentistry, we classify them based on where they are and how deep they go. Understanding which one you have is crucial for determining the right cracked tooth repair strategy.

1. Craze Lines

These are tiny, superficial cracks that only affect the outer enamel. If you look closely in the mirror, you might see faint vertical lines on your front teeth. The good news? These are very common in adults and usually cause no pain. They are mostly a cosmetic concern and rarely require aggressive treatment.

2. Fractured Cusp

The “cusp” is the pointed part of the chewing surface of your tooth. Sometimes, a piece of this cusp creates a crack. This often happens around an old filling. The weakened part of the tooth might eventually break off. Surprisingly, once it breaks off, the pain often stops because the tooth is no longer flexing. We can usually fix this easily with a crown or a new filling.

3. The Typical Cracked Tooth

This crack extends from the chewing surface vertically down toward the root. It hasn’t split the tooth into two pieces yet, but the potential is there. If the crack has not reached the gum line, it is possible to save the tooth. If it extends below the gum line, the prognosis becomes more difficult.

4. Split Tooth

If a cracked tooth is left untreated, it can eventually become a split tooth. This means the tooth is separated into two distinct segments. Unfortunately, a split tooth can rarely be saved intact. We focus on saving any portion of the tooth possible, but often, this requires removing the tooth.

What Causes Our Teeth to Crack?

You might be wondering how this happened in the first place. Did you do something wrong? Usually, it is a combination of factors and wear and tear over time. Here are the primary culprits I see in my office:

The Daily Grind (Bruxism)

Grinding or clenching your teeth, especially while you sleep, puts massive amounts of pressure on your enamel. Over years, this weakens the structure. Data Point: According to the American Dental Association, people who grind their teeth can exert up to 250 pounds of force per square inch, which is significantly higher than normal chewing forces and a leading cause of fractures.

Large Fillings

While fillings treat cavities, they don’t restore the original strength of a whole tooth. If you have a large silver (amalgam) or composite filling, the remaining natural tooth structure is thin. It acts like a wedge; when you bite, the filling stays hard, but the thin walls of the tooth flex and eventually crack.

Dietary Habits

Chewing on hard objects is a fast track to a crack. Ice cubes, hard candies, unpopped popcorn kernels, and even fruit pits are dangerous. Using your teeth as tools to open packages or tear tags off clothing is also a major risk factor.

Temperature Extremes

Rapidly changing the temperature in your mouth can cause expansion and contraction. For example, eating hot soup and immediately washing it down with ice water puts thermal stress on the enamel, leading to fractures over time.

How We Diagnose the Problem

Diagnosing CTS can be tricky. As I mentioned earlier, X-rays are 2D images of 3D objects, and cracks often run parallel to the X-ray beam, making them invisible on the film. So, how do I find them?

I use a combination of tools and history taking. I will ask you about your chewing habits. Then, I use a tool called a “tooth sleuth.” It looks like a small plastic stick with a notch in it. I will have you bite down on the stick with specific parts of your teeth. When you feel that familiar sharp zing, we know we have found the offending cusp.

I may also use a bright light (transillumination) or magnification loupes to spot hairline fractures that aren’t visible to the naked eye. In some cases, we use specialized dyes that seep into the crack to make it stand out.

Cracked Tooth Repair: What Are My Options?

Once we have identified the crack, we need to talk about solutions. The goal of cracked tooth repair is to stop the movement of the tooth segments and seal the crack to prevent bacteria from entering the pulp.

The treatment depends entirely on the severity and depth of the crack. I always aim for the most conservative treatment that will provide long-term success.

Dental Bonding

For very minor cracks that don’t cause significant pain or risk to the structure, we might use a composite resin bonding. This is the same material used for white fillings. It can seal the crack and improve the look of the tooth, but it isn’t strong enough for deep structural cracks on molars.

Dental Crowns

This is the most common and effective treatment for CTS. A dental crown is a custom-made “cap” that fits over your entire tooth. By covering the tooth, the crown acts like a splint. It binds the tooth together so that when you bite down, the tooth can no longer flex apart.

This stops the pain immediately for most patients. It also protects the tooth from breaking further. Root Canal Therapy

If the crack has penetrated deep into the tooth and irritated the nerve significantly, you might have lingering pain or sensitivity to heat. This usually means the pulp is damaged. In this case, we need to perform a root canal to remove the damaged nerve first. After the root canal, we will still place a crown on the tooth to stabilize the crack.

Extraction

I view this as a last resort. If the crack extends deep below the gum line or has split the tooth entirely, the tooth cannot be saved. In these cases, we remove the tooth to eliminate pain and infection. We can then discuss replacing it with a dental implant or a bridge to restore your smile.

Prevention is Better Than Cure

While we are experts at cracked tooth repair, I would much rather help you avoid the problem altogether. You can’t always prevent a crack, but you can significantly lower your risk.
